Location: GP Practices across Croydon

Pay: competitive salary based on experience.

Hours: Part-time and Full-Time – 7.5 hours to 37.5 hours

Work pattern: In person (GP practice–based)

About the Role:

We are looking for a compassionate and motivated Social Prescriber Link Worker to join our team on a part-time or Full- time basis. This role supports people to improve their health and wellbeing by connecting them with non-clinical services, community groups, and practical support.Working with individuals who may have complex social, emotional, or practical needs, you will take a person-centred, strengths-based approach, helping people identify what matters to them and supporting them to make positive changes.

Key Responsibilities

  • Meet with patients referred by GPs and other primary care professionals
  • Listen to patients and carry out holistic assessments to understand their needs and priorities
  • Support patients to access non-medical support such as physical activity, social groups, skills development, employment support, and help with forms or benefits
  • Develop and review personalised care and wellbeing plans
  • Motivate and empower patients to make positive behaviour changes
  • Build and maintain strong links with local community groups, charities, and support services
  • Work closely with GPs and other healthcare professionals to provide joined-up, non-clinical support
  • Maintain accurate and up-to-date records in line with NHS and organisational requirements

The Person We Are Looking For

You will:

  • Be emotionally resilient and confident working with patients who may have complex needs
  • Be hardworking, organised, and reliable
  • Be excellent at forming and maintaining professional relationships
  • Be comfortable working on-site as part of a GP surgery team
  • Be an independent problem-solver who can manage their own workload
  • Have strong functional IT skills, including use of office and administrative software

Essential Knowledge & Skills

  • A basic understanding of common issues faced by patients in primary care (e.g. long-term conditions, mental health, social isolation, financial or housing difficulties)
  • Ability to signpost and refer patients to appropriate community and voluntary sector services
  • Understanding of confidentiality, safeguarding, and professional boundaries

Training & Support

We will provide:

  • Full training for the role
  • Ongoing supervision and support
  • Opportunities for development within social prescribing and primary care

Preferred (but Not Essential)

  • Experience in health, social care, or the charity/voluntary sector
  • Experience of working in a people-facing role
  • Experience using IT systems to record and update patient information
