Work Location: County Hall, Glenfield, Leicester, LE3 8RL - although travel across the county will be required as part of the role

Are you a dedicated, flexible, reliable, and enthusiastic care professional with a passion for supporting people within their own homes? If so, then a role within our Home Care Assessment and Reablement Team (HART) might be right up your street.

About the Role

The HART service helps people to return home from hospital or prevents them from being admitted, by supporting them to regain their independence. As a HART Support Worker in HART, you will provide short, intensive reablement support to people with disabilities and those who are frail and recovering from illness or injury, focusing on helping them to restore their optimum level of independence. You will be required to motivate them to help them build their confidence by identifying their goals and providing positive feedback about their progress and achievements.

The support you provide may include helping with daily living tasks such as washing, dressing/undressing, showering, bathing, meal preparation, domestic activities, and engaging in community activities. You will also be required to support with identifying suitable assistive technology equipment and demonstrating its safe use, as well as advising on strategies for managing fatigue and pain, and preventing falls.

You will be required to work shifts on a rota that operates between 7.00am and 10.00pm including evenings, weekends, and bank holidays.

A DBS enhanced check for a regulated activity is required for this post.

About You

To apply for this post, you must:

  • Hold a Level 2/3 Diploma in Health and Social Care (or other relevant equivalent qualification) or an equivalent level of relevant experience and a willingness to undertake the diploma/care certificate after induction.
  • Be committed to delivering high standards of care/support.
  • Be able to work effectively on your own as well as part of a team.
  • Have basic IT skills sufficient to write and maintain records.
  • Be able to communicate effectively both verbally and in writing and record relevant and accurate observations.
  • Be able to work under pressure and to remain calm in crisis situations.
  • Be willing to attend relevant job-related training as required.
