Salary Band: £24,500 – £25,407 FTE per annum (pro rata)

Part Time Hours: 18.5 per week

Location: Wyre Forest, Worcestershire

Contract Type: Fixed Term/Maternity Cover (up to 12 months)

About the role

Join ONSIDE as a Social Prescribing Link Worker and work within an organisation and be part of team of professionals working with our NHS partners across The Wyre Forest Network of Independent Practices GP Practices to support local people to take greater control of their health and feel more empowered to look after themselves.

A Social Prescribing Link Worker works with individuals to understand what really matters to them and links them with non-medical support to address their needs in a holistic way. It is recognised that people’s health is determined primarily by a range of social, economic and environmental factors and a Social Prescribing Link Worker aims to support people to address and deal with issues relating to these factors.
