Full job description

The Farmhouse Surgery is currently looking to recruit dynamic and reliable person(s) to join our Care Navigation team to receive, assist and direct patients in accessing the appropriate service or healthcare professional.

The Care Navigation team are the first point of contact for many patients and visitors to the practice, therefore experience in this type of patient facing role is essential, as is experience of using SystmOne. The role varies on a day to day basis, however, responsibilities involve:

  • Providing general assistance to all Practice teams, whilst projecting a positive and friendly image to patients and other visitors, both in person and over the telephone.
  • Registering all new patients accurately and promptly onto SystmOne.
  • Dealing with general enquiries, explaining procedures and booking new and follow up appointments using SystmOne.
  • Ensuring that all urgent queries are dealt with in an appropriate and timely manner.
  • Assisting with the processing of tasks, emails and general enquiries.
  • Assisting with prescription requests, ensuring they are processed within 48hours.
  • Ensuring any correspondence, reports, results etc are dealt with appropriately.
