Greens Norton and Weedon Medical practice are looking for a dedicated and experienced Advance Nurse Practitioner. They will join a clinical team across two sites which includes x2 experienced prescribing paramedics, one ANP, one prescribing nurse and 3 practice nurses, 4 HCAs and 11 GPs (7 Partners and 4 Salaried), as well as allied health staff including SPLWs, Podiatrist and pharmacists. Greens Norton and Weedon Medical practice is based rural South Northants area with countryside views, with both sites close to the A5. The practice cares for over 14,500 patients and boasts very high patient satisfaction rates, always finishing top in the county for patient satisfaction.

The role will include being incorporated into the Duty team (x2 GPs and one paramedic). On Mondays (with two paramedics) Tuesdays and Thursdays seeing “book on day” patients with minor illness problems. The other day may be more regular nursing appointments if you choose so.

37 hours per week ideally across 4 days, but open to 5 days a week if this suits. There is also the option of opting into weekend work for vaccinating / GPEA if the availability arises at which pay will be negotiated at an enhanced rate with the partnership.

We are very much a supportive towards our staff with plenty of time to discuss patients with supervising GPs if necessary.

MAJOR Duties AND Responsibilities OF THE POST

1. Clinical Practice

To deliver a high standard of patient care as an Advanced Nurse Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.

Provide the choice of direct access to a Nurse Practitioner both in Practice and over the telephone for the general practice population.

Assess, diagnoses and treat patients with a range of acute, non –acute and chronic medical conditions.

Clinically examine and assess the patient instigating necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports and plan clinical care accordingly.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

To prescribe safe, evidence based, cost effective and appropriate medication in accordance with current legislative framework and within the post holders training and registration, to include advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.

Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies

Work with patients and their carers in order to support compliance with and adherence to prescribed treatments

To initiate appropriate and timely consultation and / or referral when a presenting condition exceeds the Nurse Practitioner’s scope of clinical practice and / or expertise.

To liaise with a doctor about patients with complex problems who need the expertise of a general practitioner.

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Implement and participate in vaccination and immunisation programmes for both adults and children

To clearly communicate the health status of the patient using appropriate terminology, technology and format.
